[FFmpeg-cvslog] Document and validate AVFrame plane pointers.

Document and validate AVFrame plane pointers.

Check that the required plane pointers and only
those are set up.
Currently does not enforce anything for the palette
pointer of pseudopal formats as I am unsure about the
requirements.
