[FFmpeg-cvslog] lavc/videotoolboxenc: vastly simplify get_cv_pixel_info

lavc/videotoolboxenc: vastly simplify get_cv_pixel_info

No longer requires per-format switch cases.

The frame==0 path was unused (and would've crashed anyway).
