[FFmpeg-cvslog] avcodec/pthread_frame: Move (init|free)_pthread() to pthread.c

avcodec/pthread_frame: Move (init|free)_pthread() to pthread.c

We have more mutexes/condition variables whose initialization is
unchecked.
Also use a proper namespace for these functions.
