The author is not /always/ to blame. Between the writing
and the printing, there's a lot of people involved, including
people whose job is to 'fix' the author's mistake (spelling,
grammatical, whatever), and they're often even less clued
than the actual authors. All specialized fields are affected.

That's why in some fields, when they publish books made
out of a collection of articles, they actually make
verbatim *image* copies of the articles ; it looks ugly
when the articles varies in typography, but at least it's
easy to do and you're 100% sure no extra mistakes will appear
in the book vs. the original articles (usually published
in specialized journals with clued editors).
