I ran tests with ffmpeg r10271 on many different DVR-MS samples I have
and the AV sync looked fine.  I did have trouble automatically
applying the patch, so I manually recreated it.  The diff file
attached shows the manual changes I made.  As long as I didn't make
any mistakes applying the changes, the testing I did looked ok for
DVR-MS files.
