The copy_permission_indicator bits shouldn't be set because 
the stream is not encrypted.

In my opinion it is not worth adding Blu-Ray support until
the muxer is MPEG-2 systems compliant. Blu-ray has further
constraints and its players are picky (especially Panasonic's).
