Please try the latest version of ffmpeg

The following should work:
./ffmpeg -f image2 -vcodec bmp -i 'test%04d.bmp' test.avi

If instead you need it to work over stdin and that still does not with
latest ffmpeg then i suggest you simply open a bug on our current
bug tracker and state there that theres a bounty for whoever fixes
it and the amount. Iam pretty sure it will be fixed quickly
