 -er 4 will break things as soon as the encoder does even the slightest
 questionable thing.
 For example (by my code understanding) the H.264 decoder will consider it
 an error if there are any unused bits in a slice.
 There are a lot of encoders that will fail such a test (though I am not
 completely sure there's not an FFmpeg bug either).
