 This has also been submitted as a possible bug in Kodi/XBMC (#16027), but
 the report has not been looked at. Kodi uses FFmpeg extensively, but also
 does some of its own modifications (as I'm sure you are aware), so I am
 not sure which is to blame at this point.

 I did perform a (partially successful) bisect of this issue with several
 Kodi/XBMC builds as the FFmpeg base they use from June of 2014 works fine
 with the same lavf-muxed files.
