 Your sample does not work (at all) with current vanilla FFmpeg (including
 the version you posted the output for): It is not possible to reproduce
 the issue you describe without patching FFmpeg, see the console output I
 posted above. Either you are using a different sample or a different
 (patched) FFmpeg but you didn't provide the patch.
