Summary of the bug:
The (QuickTime?) TimeCodeHandler is always copied to mp4, in spite of
using `-dn` and/or `-ignore_unknown`, `-ignore_chapters 1`.

As for my purpose the data stream is of no use I would expect `-dn` not to
copy it.
