I probably don't understand your issue (mostly because you 
are not posting output for your original sample that you 
had problems with and because you are using an outdated 
version of FFmpeg), but the fact that libavcodec/FFmpeg 
is unable to extract timestamps from some h264 streams 
is a known limitation.

Patch very welcome, but this apparently is not trivial;-(
