Was getting the same error and added this code:

unsigned int tag = 0;
if ( av_codec_get_tag2( av_out_fmt_ctx->oformat->codec_tag, codec_ctx->codec_id, &tag ) == 0 )
{
    av_log( NULL, AV_LOG_ERROR, "could not find codec tag for codec id %d, default to 0.\n", codec_ctx->codec_id );
}
video_st->codecpar->codec_tag = tag;
