Changes (by John Regan):

 * Attachment "fragmented-aac-tfhd.mp4" added.

 Fragmented MP4 file with AAC audio. Contains an edit list box signaling
 priming samples to discard, the final fragment uses the tfhd box to signal
 the last packet has 768 samples. Should decode to exactly 96000 samples.
