Comment (by iangray):

 I've had success using "bmxtranswrap" utility from the BBC's "BMX"
 library:https://github.com/bbc/bmx/releases

 I used bmxtranswrap to remux ffmpeg_mxf_output.mxf, and the result plays
 appropriately in Premiere (albeit with the first 2 frames omitted).

 On Windows:

 {{{
 %USERPROFILE%\Downloads\bmx-win64-binary-1.2\bin\bmxtranswrap.exe -p -o
 "output file.mxf" --clip "OUTPUT CLIP" "input file.mxf"
 }}}

 On Mac:

 {{{
 ~/Downloads/bmx-macos-universal-binary-1.2/bin/bmxtranswrap -p -o output\
 file.mxf --clip "OUTPUT CLIP" input\ file.mxf
 }}}
