You cannot expect a "real" encoder (even flash video is 
one of them) to reach an arbitrary high bitrate.
Use -qscale 2 for best quality (or "1" if you want but 
afair old MEncoder documentation claimed that "1" only 
increases filesize for no quality gain - otoh, the 
MEncoder documentation is outdated, so perhaps "1" is 
better than "2").
