Is OK to start with these?

av_alloc_format_context -> avformat_alloc_context
av_register_all         -> avformat_register_all
register_codec          -> avcodec_register

The plan is as usual:
1) define the new function and bump minor, deprecate the old one
2) s/old/new everywhere
3) drop the old version at the next major bump
